注意事项：
  • For applicant from single parent family, kindly provide divorce letter or death certificate of the late parent
    若申请人来自单亲家庭，请提供相关的离婚证明 / 已逝亲人的死亡证明
  • For applicant's parent(s) / spouse who does not have income document, kindly provide a Surat Akuan signed by the Commissioner for Oaths (Pesur uhjaya Sumpah) / Cour t (Mahkamah) - Click here to see sample Surat Akuan
    若申请人的父母或配偶无法提供收入证明，请准备一份证明信并到宣誓官或法庭宣誓盖 章 - 点击这里查阅证明信样本
  • All documents submitted (except EPF Slip, Statutor y Declaration Form and CTOS Consent Form) must be certified true by civil servant Grade 41 and above or other officials from "Pegawai Kumpulan A"
    所有提交的文件副本（除了马来西亚雇员公积金报表，法定声明表及信贷资讯同意书已 外）都必须由Grade 41以上的政府官员或 "Kumpulan A" 的官员盖章认证
  • Statutory Declaration Form must be signed by the Commissioner for Oaths ( Pesur uhjaya Sumpah) / Court (Mahkamah)
    法定声明表须由宣誓官或法庭签字盖章
